2027 Medicare Drug Prices: What Agents Should Know

Medicare drug pricing is changing in 2027, and some of the headlines could easily confuse clients.

CMS has published negotiated prices for 15 Medicare Part D drugs that take effect January 1, 2027. Some of the reductions are significant, including several widely used medications.

But there is one important distinction agents need to communicate:

The negotiated price is not automatically the client’s pharmacy copay.

A client’s actual prescription costs can still depend on their plan, formulary, deductible, drug tier, pharmacy, and other coverage factors.

What Is Changing in 2027?

CMS selected 15 Part D drugs for negotiated pricing beginning in 2027.

According to CMS, approximately 5.3 million Medicare Part D enrollees used these drugs in 2024, and the agency estimates about $685 million in beneficiary out-of-pocket savings in 2027 under the standard benefit design.

One of the most visible examples is Ozempic, Rybelsus, and Wegovy.

CMS lists:

  • 2024 list price: $959
  • 2027 negotiated price: $274

That is a substantial reduction—but agents should not present $274 as the amount every client will pay.

What Agents Should Tell Clients

The simplest message is:

“Medicare has negotiated new prices for several drugs in 2027, but your actual pharmacy cost will still depend on your plan. We should review your medications and coverage before AEP.”

That keeps expectations realistic while creating a natural reason for a plan review.

Review More Than One Prescription

Agents should avoid focusing on one high-profile drug alone.

A client’s total prescription costs can be affected by:

  • Formulary placement
  • Drug tiers
  • Deductibles
  • Copays or coinsurance
  • Preferred pharmacies
  • Prior authorization
  • Other medications

The best plan for one prescription may not be the best plan for the client’s complete medication list.

Start Preparing Before AEP

Agents can begin now by identifying clients who use the affected medications.

Consider:

  1. Updating medication lists.
  2. Confirming preferred pharmacies.
  3. Tagging affected clients in your CRM.
  4. Scheduling prescription reviews before AEP.
  5. Comparing 2027 plan and formulary information once available.

The goal is simple: help clients understand what the new pricing actually means for their coverage.
